For some reason in the Desktop Icon is not the System Icon ~system.ico but
another icon on a button.

What might cause this?

Another piece of the puzzle is that it is a one window app with no frame.

That really shouldn't make any difference. The shortcut icon will
always, by default, be the first icon resource within the EXE. Did you
use the project "Application Icon" setting to specify which icon to
use?

Hmm. If you right click the shortcut, open properties and then "Change
Icon" is the top/left icon the icon you specified as the Application
Icon?

No the correct icon is to the right of the wrong icon.

The icon on the Clarion exe is likewise not the correct icon so it doesn't
appear to be a SB problem.

There is a difference between the -global- "Default icon" and the
-project- "Application icon".

The default replaces the triangle icon on all windows without an icon
attribute and the application icon is the first icon the linker adds
to the EXE so Windows can find it.
