What is the difference between an installer that needs to be save to the
hard disk before running and on that runs and installs over the internet

When you are downloading a setup.exe file then you can "RUN" it and the
program will execute (after it made it into a temporary Windows folder). If
you "SAVE" it then a copy is put on your hard drive in the folder you
specify (or the default) and then you can go there and RUN it later. In
many cases it is best to SAVE so you have a copy of the file.

You can create a Web Install with SetupBuilder, but I think that is not what
you are referring to in this case. As I understand it, you mean the
standard "Run" and "Save" feature in Windows (see attached screenshot).

If the user clicks "Run" then Windows will download the setup.exe file into
a temporary Windows folder, executes it and removes the original file.

If the use clicks "Save" then a copy is put on the hard drive in a folder
your customer specifies (or the default) and then your customer can go there
and RUN it later. The advantage is that he has a backup of the original
setup.exe.
